Self-Hosted VideoPress Install – Can't Link?

Hello all,

I’ve got a self-hosted wordpress blog where I’m gone through the steps of purchasing and installing the VideoPress plugin – as per this video:

However when I click the VideoPress icon to insert a video into a post, I do not receive a login page to link my self-hosted blog to my WordPress account. I simply get a blank window, see reference photo below:

Any ideas?

Also, I have a custom theme via ThemeForest. I am suspicious that this may be the root of the problem. Yet when I switch to the default theme, the problem still exists.

Plugin-wise, I have Jetpack and Password Protect installed.
