Score Submission (Plugin Needed)

One of the websites I run has a game that it’s members “compete” with each other at. The site has recently wanted to start offering some “rewards” for it’s top scorer of the week, and they’d like me to implement a simple score submission plugin into the site. I’ve searched and have found nothing that will quite do the job.

Really, what I need is very simple. The member will play the game and then submit his/her score (along with name and which level was played) via a simple form (widget or page based). Then, the plugin (that I’m hoping exists) will compile the score in a simple database (adding the date of the form submission).

Anyone know of anything like this? I’m sure there’s something out there, probably far more capable than I need that I could just scale down for this purpose. I’ve used Membership Database before and really like it’s database, but I see no ability for it to allow visitor submission of data.

Any ideas?
