Request Form

Hi I am totally new to wordpress and will be building a second site that will include a flip file of the entire magazine as they come out. Now my question is I want to add a contact form gathering the relevant information for anybody who wants a hard copy sending out. Can anybody advise how best to do this?
Appreciation in anticipation of being directed in the right direction. I have tried searching in plugins but dont quite seem to hit on the best keywords to find what I want.
