Related posts thumbnailed hack or plugin?

    I am looking for the best way to do what mocoloco.com has on their site- related posts in thumbnails with the number of comments displayed on top of the thumbnail.
    I tried a few different plugins but none seems to be able to do anything similar.
    What needs to be included is a thumbnail of the first image in the post, and a link to the post, the rest is optional.
    How do you achieve this same effect in WordPress?
