Registered Member Plugin

I am looking to find a plugin that will have a member register on the site then send an automatic email to the admin asking the admin to approve the member. The admin will use a form that gives them permissions to certain pages on the site as well as input additional information that may be displayed as a drop down box or check-box.

It would be great if this plugin would have features such as lost password, modify my details, etc…
