    What version of WordPress? What version of the plugin? What is your theme and theme version?

    WP 3.9.1 and Event registration ver 6.00.31 and the theme is iFeature Pro 5 ver 5.1.2.5

    This issue is related to your theme. If you change to a WordPress default theme, you will see the issue go away. This has to do with the theme loading the shortcode twice (you should not have more than one shortcode on a page!).
