Question, forms with upload func

    I got a question, How can I make a form, on my blog, so can my regi8stered users and VISITORS can complete the form, and upload a photo?
    I want them to upload a photo from their pc, on my blog, and into my pc.

    Anyone knows how?
    Thanks in advance.
