Problem WordPress Forms

I installed the plugin: WordPress Forms, but in trying to create a new form, the screen does not load anything.
http://www.ojonaranja.com/wp-admin/post-new.php?post_type=formThe WP version is 3.0.1 and the plugin version is 0.2.6
PHP is 5.01 version.Help me please!
