Primary/Alternative Contact Select Option Not Showing

The select option for work/home etc shows briefly when page is loading then disappears and the options cannot be selected. This also causes the form to be unsubmittable when phone number is a required option. You can view the form here —> http://www.caspervain.com/team-member-application/
