If I have stored images in monthly and year based folders how do I prevent hotlinking? I understand that one needs to put .htaccess file to prevent hotlinking in the folder where images are stored but i have images stored in several fodlers.

You can apply this http://perishablepress.com/press/2007/11/21/creating-the-ultimate-htaccess-anti-hotlinking-strategy/ to the main .htaccess file in WordPress, and it will block for all subfolders too.
