post not updating

I’ve added a photo (last week) and link to YouTube video (today) to a post, neither are showing up on the live site, and only the photo shows up in the preview. On the edit post page I have the code view showing up correctly, but on the visual view I see a yellow box for the YouTube video and no photo.

Why would there be 4 different versions of the same code, and how do I get the code to appear?
