   Hello,
   I would like to know if there is a poll plug-in that has an option for the user to enter his email when submitting his votes.

   Meaning it would have the usual questions, then right above the SUBMIT button, there is a (required) field to enter an email and then submit the votes.

   The email entered then gets sent to a specific email.
   Is there a plugin like that?
