[plugin:wp-user-frontend] is not displayed the attachment /uploading box

I activate the plugin wp-user-frontend in that admin bar i enable the attachment still i call the plugin by shortcode [wpuf_addpost]
only the tilte with description and the attachment name only displayed not the (upload box) anyone help please………..and i need to upload my videos in the post by front end any one….. i create the uploading option by add the code in attachment.php the code is to be

i upload the video but not displayed …..in the post can anyone know please help?
