i'm looking for something specific. i have tried out and tested most of the polls/surverys here. not sure if what i'm looking for is either of the two or a form, maybe?

this is just an example of what i'm looking for.
i want to ask a question and have visitors be able to submit their name, location, and answer and have it show; rotating like the example of tonic.com.
ex: (q) what inspires you?"
(a: i'm inspired by...) answer inserted by visitor...
(a: name) answer inserted by visitor...
(a: location) answer inserted by visitor...
i'm just wondering if there's a plugin/widget anyone knows of? perhaps a jquery code somewhere? any help would be great.
