[Plugin: YIKES, Inc. Easy MailChimp Extender] Short Code?

Hi There, thank you for the great plugin 🙂 I’m wondering though if there’s a way to place the email line & name line next to one another for page layout purposes with shortcode & add ” : ” behind each word & a title above it? Here’s a url to see what it looks like now:
