   The generated form (either in div or table format) does not show form field labels.

   The div version of the form does have labels.

   Example:
   

   <div class="yks-mailchimpFormDivRow">
   	<label class="prompt yks-mailchimpFormDivRowLabel" for="bb0327d809-LNAME2">Last Name</label>
   	<div class="yks-mailchimpFormDivRowField">
   	<input name="bb0327d809-LNAME2" class="bb0327d809-LNAME2" id="0-bb0327d809-lname2" value="" type="text">
   	</div>
   </div>
