I am displaying 3 ads at a time, set to random. I have equal number of ads for each slot, and every refresh of my browser, I get the default "advertise here" image in one of the 3 slots. How can I get it to ignore that default ad if I have well over 3 ads to fill all of those slots? Is that a bug?

It may be a bug, though it depends on how you look at it. WP125 was originally intended to only have 1 ad assigned to each ad slot. If you're putting more than one ad in a slot, I can't really say how it would behave.
That said, I'm planning a major rewrite of the plugin that I will hopefully be working on over the coming months. It should be more robust.
