hmmm … that sure looks like a bug.
could you test to see if the problem is also present when adding a video in a page/ post without a widget? and with a vid in a page/post and the widget?

* provide me with info on the browser/ os you use?
* provide me with info on the blog you’ve found this bug on (wordpress version, the theme you’re using and if available a URL where I can go hunt bugs for myself)?

update (we mailed extensively): justin is on ubuntu+firefox (as am I) and confirmed this works on my test-blog. so the problem seems to be related to his installation (wordpress version/ theme/ plugins). waiting on further feedback from him about that.
if anyone else experiences this problem as well, do contact me or reply in this thread, so I can investigate further and if need be issue a bugfix.
I have the error too, works on mac osx and chrome but mac osx & firefox (18.0.2) gives the ‘c.getElementsByTagName is not a function’ error
