   Hi all

   Does anybody else have the FORM appearing above the Header?
   and in the body?

   Unsure if this is wp3.3. issue.

   Some other plugin may be conflicting with this one. Try disabling other plugins.

   i have the same problem, but i need this other plugin too:

   perhaps you could have an eye on it.
