[Plugin: WP Survey And Quiz Tool] Adding Checkbox in Contact Form

I was wondering if anyone knows how to put a checkbox selection option in the contact form portion of the survey. I’d love to be able to ask something like ‘Would you like to sign up for our e-newsletter?’, with a checkbox when I’m collecting contact info.

I’ve tried posing the question with radio buttons, but that function doesn’t seem to work for me. When I select Radio Buttons, a Text Area box just appears.

Any help would be greatly appreciated. Thanks!
