I have this problem and this is my structure.
Actually, I think my problem is a little different. The site functions correctly, but the only page that caches on the entire site is the home page. Not even the other pages (as opposed to posts) are cached.
The problem goes away when I change to PHP-based caching. My wife's blog works fine with mod-rewrite method, however she is using a standard permalink structure.
Woops, I take it back. PHP-based caching is not working either. When I activate preload, I get a nice collection of cached pages. But after a while, the list just reverts back to my root page only. The only real difference between my site and my wife's is the permalink structure.
Try the development version on this page. Hopefully that should fix it.
