The PHP mail subject became a nightmare for me. When php mail is sent from contact form SPF gets always failed and became a headache for me until I found the correct details. It took me some time to figure out the correct ways of doing things...
One suggestion: I think the password type in your email setting page should be corrected to password which now is text.
