Can't seem to find any info at all as to usage. Shortcode? template file? I don't see anything in the settings that might place it on a page. Any help greatly appreciated.

That plug-in hasn't been updated since 2008 and may not even work with the current version of WordPress. I would not use something so out-of-date for critical functionality like a contact form.
I would use something more current, for example Contact Form 7 is easy to use, kept up-to-date and has good documentation:

Thanks for the advise. I had to dig to find the shortcode. I tested it, It works, It was recommended only yesterday by an admin for WP Store Cart as one they knew worked with their plugin.
Anyone else looking for the shortcode can find it in the plugin's read me in the WP theme editor. -
