[Plugin: WordPress SEO by Yoast] Using data in a custom meta box in post title?

I watched a video that talked about being able to use %%cf_<custom_field>%% in either the title or the meta description. However, I have a custom post type that uses some custom meta boxes. Is it possible to use the data from those as well?
