I've been told that (post penguin) putting keyword in title, url, meta keywords, meta description, 3-5% in content, etc., is over optimizing and we will get penalized by Google for following the guidelines. Do you know, is this really the case?

I just started using this plugin, but after looking at a post page analysis for my chosen focus keyword, I noticed that it wants me to put this keyword in more places. I just got through "optimizing" another of my sites by REMOVING keywords, which seems to be what many SEO experts are advising. I'd really like to know if anybody has any information about this, before I make any more changes. Looks like this plugin wasn't updated to reflect the "new SEO".
