   On activation, I get a "You do not have sufficient permissions to access this page." message for /wp-admin/tools.php?page=wp-reset

   Logged in as administrator, wordpress installed in a subdirectory, plugin zip uploaded via plugin manager.

   The page should be wordpress-reset and not wp-reset. Also, try using the newly released 1.3.1.
