That happens when the browser window is 'too narrow'. I know the interface needs a facelift, it hasn't changed much since 1.0, and I have a big monitor, so I didn't notice it until it was too late. It does work on an iPad in landscape, though. There isn't much you can do about it, other than make your browser window wider.
The screen shot was taken only of the Form Manager panel in full screen. Again, the browser is in full screen mode. I am running at 1024 x 768. I've never experienced any other WP plugin with this problem. The layout is just poor design for any thing else but a wide monitor or an iPad.

You are right, the design is bad. The interface hasn't changed in any major way since the first version of the plugin. Honestly I listed the iPad as an example of a small screen on a mobile device, but I understand 1024 x 768 is not exactly tiny. Making the interface nice on smaller screens is on the list of major changes for version 2.
