[Plugin: WordPress Download Monitor] Download Monitor – Logging not working

I am using Download Monitor in conjunction with ‘Email before download’ and ‘contact form 7’ on our site. The page that I am having trouble with is: http://www.joeyssong.com/kids-music. Everything is working: You can enter your name and email, you get the success message, are presented with the link to download the song, email is sent with a link. But when I go look at the log of Download Monitor, it is not capturing the transaction.

Any help to resolve would be greatly appreciated.
