[Plugin: W3 Total Cache] .htaccess rewrite rules don't apply in disk (enhanced) page cache mode

When I turn on the disk (enhanced) caching mode, the cache files get created when a page is accessed, but subsequently they are not accessed – in fact, the website behaves exactly as if there was no page cache. Instead of “redirecting” to the cached files, wordpress simply goes about executing it’s stuff.

I tried playing a bit with .htaccess, and I am 100% sure that for some strange reason, the rewrite rules created by W3TC are not applied.

Has anyone ever experienced something similar? If so, how did you fix it?
