[Plugin: Visual Form Builder] text field info missing when form submitted

When using visual form builder, the last two questions on the form require a typewritten answer. When filling in text forms and submitting them, everything works perfectly except for these last two questions. They both come back blank. I’ve tried using them as a text form and as text but no luck with either. Any solutions?
