   When using visual form builder, the last two questions on the form require a typewritten answer. When filling in text forms and submitting them, everything works perfectly except for these last two questions. They both come back blank. I've tried using them as a text form and as text but no luck with either. Any solutions?

   Yep, same problem here. Very irritating. Otherwise nice piece of plugin, but this sucks.

   Any ideas?

   And at the same time I realised what was wrong. The name of the field was too long. Now everything is OK.

   I had the same issue. It seems questions need to be 9-10 words and under. I had more than 10 words in a couple questions and they would not submit a value in the form.

   Shorten the text for the form question and that should fix missing field values.
