    Not sure exactly what you mean. Perhaps a description only field?

    I mean a textarea where we pre-fill in with some text (that will be presented it before the submit button), something along the lines like:

    By sending out this form you understand that your request will be answered by our support team within the next 4 working days and no sooner than 24 hours.

    or

    You will be provided with a free quota based on the information you give us. This is by no means binding should we decide to accept the repair work.

    Underneath it there should be a checkbox that would be required.
    Just couple of thoughts. It could actually be used for many other things.

    In a way, no, that’s not something that I can do.

    I can add a description field. However, you can add a checkbox and make it required.

    Once the description field type is added, then this will be something that you can do.

    If they want yo send the form, they would HAVE TO AGREE by checking the box I will also place underneath it.

    Bonus Network questions….
    1. How does the plugin behave on networks? Should it be network activated? Are there any differences if not?
    2. Does it create any tables on the database? If so, one for every site or just one for all?
    3. Does the csv file contain data from ALL sites or for a specific one?

    Yes.

    I’m not sure exactly how it functions on multisite because I haven’t tested it in that scenario, but the plugin does create new tables. My guess is that it will create new tables for each site that you install it on. That will mean that the CSV is just for that site. I can’t confirm this. If a multisite user would like to chime in here, that’d be great.

    I have a multisite network of 5 sites and I can confirm that a created form only works in the site you created it in. I was just looking for a way to copy it to other sites instead of having to create it again. Any ideas?

    I made a terms of use field by first inserting html saying, by clicking the checkbox hereunder you confirm accepting our terms of use
    then I insert checkbox that is required with text, terms of use. I think that works fine.
