[Plugin: Visual Form Builder] Checkbox Alignment

Having issues with Check box Alignment. Here is link to page. http://bikerdown.org/1test-2/
If I float:left; then the Check box does not line up with text.

I need to have a Check box then the text.

Also is there away to only have them choose one check box?

And if this is a stupid question I apologize a head of time. I am new at this.
