   This plugin seems to do exactly what I need it to do, but one issue -- I'm using a Theme My Login page. Where can I change your redirect to the default WP login page to my themed login? Thanks.

   I have added three hooks for the next version:

   uam_login_url - a filter for changing the login link if multiple posts are shown
   uam_login_form_url - a filter for changing the login form link if a single post/page is shown
   uam_login_form - a filter to replace the login form completely

   You could contact the plugin author and ask if he will implement this or do it by yourself. You will find the current development version here: http://downloads.wordpress.org/plugin/user-access-manager.zip
