A previous version of Tweetbacks had an error which caused inadvertent spamming of random tweets for some people, but it was fixed very quickly. I don't know of any reports of spamming happening with the current version.
I've looked at the code pretty extensively and I'm not much of a coder but the whole thing is only about 500 lines long. There's definitely no intentionally buried back doors or anything like that. In addition, Joost de Valk - the author - a very respected figure in the WordPress world and has written some of the most frequently used plugins there are.
In short: I'll eat my hat if it's true.
