   I installed this plugin, and went to the login page to see how it looked. The login form box had instead been replaced by this text:

   [theme-my-login]

   Fortunately I was able to go BACK in a separate browser program and window under different login, and de-activate the plugin.

   Except that didn't solve it.

   So I went through FTP and manually deleted the entire plugin.

   That still doesn't solve it.

   I now have NO login ability whatever, only the text "[theme-my-login]" where my login form should be, and must wait for my server company to get around to helping me.

   If you deleted the plugin, it will revert back to the default login. Access directly wp-login.php.
