When the site is upgraded to WP 3.4, Theme My Login only displays the following:
"Your messange hasn't been sent.
5 attempts remaining."
The only way to access wp-admin is to rename the plugin via FTP then log in using http://sitename.com/wp-admin. Renaming the plugin back and then reactivating the plugin produces the same results.
Yes it is, sort of. I just found out it triggers a flaw in my theme. No wonder it didn't get any better when I disabled all the other plugins! Theme is Zenon lite, BTW.

can you change the theme and see whether its still happening or not?
I found the problem. It is with contact.php. I am using CF7 anyway so I just deleted that template file. The Theme My Login plugin now works as expected.

Same Problem here but receiving error 404. deleting contact.php did not work. It did not work to disable other plugins and update theme (colorways free). renaming them my login-folder helps me to login via wp-admin, so i think there is a problem with tml.
