   I am looking for a plugin that allows me to add polling/questionnaire that ties up with reader comments.

   I have reviewed a few plugins like wp-poll and democracy, they are good but not quite doing what I need.

   I need to get the email address of the voters, while these available plugins that I came across only log down the IP address.

   I need to determine which user (unique by email address) votes for what.

   Anyone can help? Appreciate that very much!!
