   Hi, Does anyone know if there's a plugin where people can "tell their story" by filling out a form? There's no need to register or log in and its completely anonymous ( i.e like on http://www.herpes-terminator.com/tell-your-herpes-story.html).

   now - does anyone know how to get this plugin working? I tried their forum, no one replies. I tried and looked at every option where you have to prefill, I just don't know how to actually get it up. do I need to start a new page (like when you do a forum?) and then paste html code? I could not find any info on that and also no html code.
