[Plugin: TDO Mini Forms] Images are only posting as a link

Images are only posting as a link. I am sure it is some kind of user error, but I have tried lots of combination with no success. What would be the standard setting for the upload widget to create a photo on post? Thanks in advance for any help on this subject.
