With the new version of this plugin (and using WP 3.0), I can now drag & drop the widgets in the Create section, but after I make changes to my form they don't "save", it goes back to the default settings with only the Content widget.

Me too, but only with Custom Fields, I Agree, Who Am I, Append to Post Content and Upload Files widgets.

Same here. When I drag 'Upload Files 1' over to Your Form and hit Save changes, it's gone.
I've seen this before with plug-in's that didn't play well together. One of the plug-in's that gave me problems was Event Calendar. I've had issues with other incompatible plug-ins as well. Try turning them all off then back on one at a time and seeing which ones don't like each other.

Awesome plugin, can't wait for the fix. For me, it's the Who Am I and I Agree widgets, they just won't stick.

I've fixed this in v0.13.9, just released. Issue with new JQuery's seralize option in sortable.
