[Plugin: Sweet Captcha] Layout breaks when clicking to "reset" captcha

For some reason when I click “reset” on Sweet Captcha it breaks my page layout and pops up a 404 page WITHIN the Sweet Captcha box, right underneath the icons. I’m using latest versions of WP and the plugin, and installed contact form 7 to see if that had anything to do with it.. I’m also running latest version of BuddyPress and Genesis (with Genesis Connect BP plugin). Any ideas what might be going on??? Thanks!
