Hi
Nice plugin - would be good to see ability to add images for rating etc without hack, but my main question is whether you can add an 'Other' free text field to the bottom of a group of radio / checkboxes etc for more specific answer not covered by choices?
If not would be a great addition for future release.

Good point , about adding the 'other', we'll see if we can come up with something that is easy and won't clutter the interface too much.
As for the images, how do you mean?
Have and image for the question or option or both?

That would be great.
Regarding images it would be good if you could:-
1. upload an image and allow users to rate - eg 1-10
or
2. Upload a selection of images and use radio to select favourite etc
This could perhaps be done of you could add html next to an option (so then it doesn't necessarily have to be an image, it could be a css box showing a colour etc).
Other than that there appears to be a bug on the form responses. No matter what you set on a new form it seems to only take that of the 'default' form. Also sometimes the mandatory field sometimes seems to stick and you cannot remove it.
It's defo one of the better survey options out there though :-)

1. This is sounding a be niche that won't fit will in a generic plugin survey plugin. We can't give everything away for free, as this is how we make our money.
2. Theoretically is is possible to put html in both the question and the options, but as any html is removed so that they can be stored correctly in the db it must also be accompanied some identifying text. This isn't exactly a feature its just something we haven't spent too much time on preventing.
3. We'll look into the responses. Was it all of them or just a few in particular?
This was tested, but nothing is 100%
4. Regarding the mandatory fields sticking, were they sticking on or off? also was it particularly any type?
Some types are forced to be mandatory, so as name,email or captcha.

Appreciate you need a return on your work! i'd be interested in getting your details if you more generally do plugin work.
'Other' really would be good to see as this seems to be the main thing missing - I have hacked used jQuery and CSS to add images to questions. It's not ideal, but it's decent enough for now.
The fields sticking as 'mandatory' were just general fields (radio select question in this case). I had to delete the question and replace it in the end. Luckily you can put a new ID in on new items to put them back into the right place!

The 'Other' something that we want to include in a future release, but like I said, its a matter of adding it , but keeping the interface simple and intuitive. Out first thought had to be discounted because it it would only work for english forms. But be sure we are working on this.
We've managed to recreate the problem and are looking into a fix for this.

I came here looking on how to add "other" with a text field, semi important option, hopefully you can find the right way to do it and still keep the plugin as clean and slick as it is.

This will be considered for a future update. Though development of out free plugins is going rather slow at the moment due to our workload.
