[Plugin: Subscribe To Comments Reloaded] Comment box & sidebar both disappeared. Help pls!

I just deactivated STC and installed STCR. Default permalinks looked right. The subscription box has disappeared on my pages, as has my right sidebar. Here is a typical page:
