Add A Field to Simple Modal Contact Form (SMCF) (2 posts)

    Hello,

    For awhile now i have been trying to add 2 new fields to my Contact Form. I have been unsuccessful for over a week now. I am super frustrated now.

    I have been searching for awhile now, and all i have been able to find out is to go to the
    simplemodal-contact-form-smcf/smcf.php page under editor
    then find the 'function footer()'
    and the under the '$form' i will be able to add.

    well i can't find a single '$form' under anything. All i have done successfully was changing the name of one of the fields *---*

    Any help would be awesome. Please help asap

    Below the form, you can work as usual...

    Example adding a double radio buttom field..

    <form action='" . $url["path"] . "/smcf_data.php' style='display:none'>
