Great plugin, loving this. Esspeccially in combinating with yoast.com's article: http://yoast.com/affiliate-links-and-seo/
But It doesn't seem to work without flushing the rewrite rules over and over. What I mean is when I add flush_rewrite_rules(); within the function register_post_type(),where it creates the post type, it is working. But this function is not good for performance as you know.
Have any idea why it is not working without the flush_rewrite_rules() function?
I have updated the permalink structure and still no luck.

Had a similar issue. After I saved the link I went to view it and got the page not found error. I'm running W3TC so I cleared all caches and tried again and it worked.

Thank you for your reply. Unfortunately I don't have this plugin running nor another cache plugin no my test environment. So this is not the problem.

In the FAQ, you'll notice that the instructions say that you have to visit the permalinks page and save them. This flushes the rewrite rules.

I had this issue with the 404 as well and resaving my permalinks fixed it. However, it happened again and I only knew about because a user was nice enough to leave me a comment and let me know. Resaved permalinks again and it's fixed.
This begs the question...why? Why would it have broken again?
I use W3 Total Cache and I often find myself "emptying all caches" when testing different things. Could this be the cause? Should I get in the habit of also resaving permalinks?
