Go to your application: http://www.facebook.com/developers/apps.php
Click Edit settings.
On the tab labeled “Web Site”, you have put incorrect information into the Site URL or the Site Domain field. Fix that.

I… cannot believe… that I made that mistake.
I swear I double checked my URLs billions of time. I trawled the internet like the Troll under the bridge.

A new problem, now the application works but the Facebook Fan Box is down instead. it was appearing fine until then.
Hard to say what the issue is without seeing the site. What’s the URL?

I’ve read through more of your other comments and such, and here are some information that might be helpful.
I’m using the Gazette theme by wordpress, seems like other users using that is rather buggy as well.
The comments function doesn’t seem to work either and I’ve read through your suggestions (can’t even find the function code). On top of that even though it connects to facebook it still requires registration.
And… the Facebook Connect widget is just a greyed out text.
I seriously stink at this. Sorry dude.

I can create a temp admin account if you need to look into the site more deeply.
FYI – There has been an ongoing problem at facebook regarding the API key for several weeks now so maybe it is related to the problems you are experiencing with Simple Facebook Connect. Here is the thread and you can also go to the bug thread from there as well. Lot of angry Developers losing their minds. I feel their pain.
