Looks like development was stopped and it does not work with latest WordPress version. Nobody seems to reply questions here for years. I managed to find another plugin doing the same job (and even more). If you were interested in this plugin, try http://wordpress.org/extend/plugins/better-seo-slugs/ instead.

This plugin works up to the latest version of WordPress and is activley maintained.
