I tried to insert a few links but while the URL is created in the background, the UI with the three fields (url, title, keyword) does not disappear, as expected, and the URL is not inserted in the text. At least not in the visual edit mode. What gives?

Really not sure what's going on. This plugin has been tested in 3.0 up in most browsers (including IE6).
Have you verified that the URLs you're generating from a post are also showing up in the YOURLS admin area?
The fact that Thickbox isn't going away indicates to me that a proper Ajax request isn't being sent back. It's impossible to troubleshoot without actually logging in to your back-end.
