[Plugin: Page Flip Image Gallery] Can not upload images

I can not upload images or add a page to a book. It takes forever to load the page then I get a blank screen with an error message: PHP has encountered an Access Violation at 0188B21F. I have looked tried a number of fixes from Google but nothing seems to work.
