I am using your plugin and mailchimp.
I am not interested in the widget or the signup checkbox below comments all I need is a signup form inside a wordpress page.
in addition to NAME, EMAIl I would like to give subscribers the choice of their preferred newsletter format as does the mailchimp form: HTML, text, mobile.
any hints on how to do that please?
